To reduce the stigma associated with being HIV positive in prison, identify HIV positive inmates within the prison system and encourage them to begin treatment in prison, and ensure linkage to treatment and adherence to treatment upon and after release.
Position requires a BA/BS degree in a health-related discipline and/or three (3) years’ experience in Health Education. Demonstrated ability to study information so as to determine, define and comprehend its elements. This position requires establishing rapport with inmates and building relationships with service providers. Bilingual Eng/Span preferred. Candidate must pass the Dep. of Corrections clearance check.
Collaborate with Department of Corrections and Community Supervision (DOCCS) medical staff to ensure an efficient and regular referral process to the Positive Pathways Project of HIV+ inmates known to DOCCS and not on treatment and with Criminal Justice Initiative (CJI) funded Transitional Planner to facilitate the provision of services and avoid duplication of effort.
Utilize motivational interviewing techniques to carry out targeted in-reach to HIV+ inmates known to DOCCS and not on treatment.
Coordinate between inmate, DOCCS medical staff and community medical provider to ensure transfer of medical information, e.g., comprehensive medical summary, to client preferred community medical provider.
Actively link release to Positive Pathways Advocate in geographic area of discharge.
Promote enrollment/enroll in the ARTAS intervention.
Collect and report data on all funded interventions/services, e.g., type of service, length of time, outcome, etc., as well as referral activities.
Maintain client files and ensure confidentiality/security of files.
Carry out strength-based case management activities to support clients during the re-entry process.
Perform standardized follow-up activities with clients, including those lost to care.
Collect and report data on all funded interventions/services using Pathways Advocate Encounter Log and additional data collection instruments to be provided by the AIDS Institute.
